Health Center

The Health Center retains medical records for seven years after graduating from the college. Immunization records are kept for ten years after graduating. If you would like to request copies of your medical and or immunization records, please submit a written request with your name, graduation year, what records you are requesting (please be specific), include a fax number or a mailing address and to whose attention these records will be sent to. Your signature is required at the end of your request.

Records less than five pages can be faxed, anything over five pages must be mailed.

The address, www.myhamilton.edu, printed in the health information packet, and used obtain your student identification number and complete the health insurance waiver is incorrect. The correct address is https://my.hamilton.edu

You can also use your Hamilton ID and password to sign in to the Health Insurance Waiver page and view your student identification number.  Your ID number will appear in a blue box on the right.
