Health Form Submission
August 1 Deadline
If the health form will be sent after the August 7 deadline, please email Francine Vaughan at fvaughan@hamilton.edu with the students name and reason for the late submission. Please review the health form before submitting to make sure it is acurately and entirely completed.
To schedule, reschedule, or cancel an appointment, please call 315-859-4111.
Students who do not cancel a scheduled appointment before the time of the appointment will incur a $10.00 fee for a "No Show" appointment! To avoid the charge, please call and cancel your appointment.
