Hamilton College requires a completed health history, physical examination and an immunization record to be submitted on admission to the College. This form gives us appropriate information and background upon which proper medical care can be based. Continuing care is contingent on receipt of this completed form.
Class registration cannot be completed until immunization records are in compliance with New York State Law and required health form documents on file per Health Center policy.
Health Form Submission after August 1st Deadline:
If your health form will be sent after the August 1st deadline, please email Francine Vaughan at fvaughan@hamilton.edu with the students name and reason why the form will be late. Please review the health form for accuracy and completion of all required information before submitting to the health center.

Students who do not cancel a scheduled appointment before the time of the appointment will incur a $10.00 fee for a "No Show" appointment! To avoid the charge, please call and cancel your appointment.
