In an effort to give employees access to benefits not otherwise available through the College, Hamilton offers dental and vision plans in which employees pay the full cost for all benefits selected. Premiums will be deducted on a before-tax basis. Dental coverage is provided by Ameritas (select "Account Access" at the top right of the screen) and vision coverage by Vision Service Plan (VSP).
Eligibility for voluntary benefits is the same as for the College's medical insurance (a full time or half-time or more employee). Dependents eligible for coverage are spouse/domestic partner and unmarried, dependent children to age 19 (23 if full-time student).
Reimbursement for dental services is made based on a schedule of benefits with a maximum benefit of $1,000 per calendar year per covered individual. You may use any dentist you choose. The benefit paid will be the same whether you use an in-network or out-of-network dentist. However, your out-of-pocket expense is generally less with an in-network dentist.
The eye care plan features the VSP Provider Network. Many services provided by an in-network doctor are covered 100% after applicable copays ($10 for exams, $25 for frames and lenses). Services provided by an out-of-network doctor are paid according to a schedule of benefits. Review the Benefits Summary, Policy Summary or visit the VSP website for more information regarding coverage for services by both participating and non-participating providers or to file an out-of-network claim online. Set up a login at VSP's website for a customized view of your benefits and providers. To search for providers at the VSP website without a login, enter your zip code on the first page and, on the second page, choose "Signature" under the Doctor Network dropdown on the left.
Except for certain change in status events, participants may only make changes to their enrollments during Open Enrollment in November with a January 1 effective date.
