The following list was developed to make it easier for you to know the actions you need to take to elect your benefits for 2013.
Review the 2013 Benefits Information update, 2013 Benefits Guidebook, and the Excellus Enrollment Booklet and complete enrollment forms as follows:
Open enrollment meetings are scheduled for Thursday, November 8, at 9 a.m., 11 a.m., 1 p.m. and 3 p.m. and on Friday, November 9, at 11:30 a.m. and 1:30 p.m. in the Sadove Student Center Conference Room (112). The focus of the meetings this year will be a review of the new Excellus medical insurance plan. ALL FACULTY AND STAFF ARE ENCOURAGED TO ATTEND.
Please keep in mind that after the conclusion of this open enrollment period you may not change your insurance and FSA elections unless you have a qualifying event and the change you are requesting is consistent with the event. Qualifying events include a change in marital status, dependents, or your or your spouse’s employment status (for a complete list, please see page 2 of the Benefits Guidebook). If one of these events occur, you must notify Human Resources and complete applicable forms within 30 days of the event or you will not be able to make the change until the next open enrollment period.
If you have any questions related to open enrollment or benefit coverages, or need assistance completing the forms, please contact Luann Zaleski, ext. 4689 or Samantha Campione, ext. 4042.
Completed forms must be returned to Human Resources by Friday, November 16 (or sooner, if possible)! Thank you.
